At Cancer Care Specialists we recognize that your time is valuable.  In an effort to streamline your appointment.
Please Arrive Early for your Appointment

To help our doctors stay on schedule and provide each patient with the attention they deserve, please arrive before your scheduled appointment:

  • New patients: arrive 30 minutes early

  • Returning patients: arrive 15 minutes early

If you arrive late, we will make an effort to see you but you may have to wait, your visit may be shortened, or you may need to reschedule.

New Patients

We request that you arrive at least 30 minutes before your appointment time.  This will allow us time to prepare your chart and get any additional information we may need. If this is your first time seeing a Cancer Care Specialists provider, please complete your Phreesia paperwork (emailed and sent in a secure text message-at the time of scheduling) prior to your first appointment.  If you prefer to complete the forms at the office, please plan to arrive 45 minutes prior to your appointment.

Returning Patients

It is very important for us to have the most up to date information.  As such our office requires that all patients complete an annual update packet.

Controlled Substance Policy

With the safety and wellbeing of our patients and the community in mind, we have changed how controlled drugs are prescribed and monitored. This effort includes our Informed Consent and Controlled Substance Agreement between the patient and his or her provider, reflecting a joint understanding of what is required for a controlled substance to be prescribed and used safely by a patient. 

This agreement is part of our efforts to combat the negative effects of controlled drugs in the communities we serve.  Cancer Care Specialists recognizes that such agreements – and the requirements within the agreement – may be inconvenient for some patients.  However, it is an important way for CCS to protect both the patients and the communities we serve from the negative impacts of these drugs. All of the Cancer Care Specialists providers and staff are committed to partnering with patients and we appreciate your understanding and cooperation as we work to combat the growing opioid epidemic in our community.
