Our referral process is designed to ensure your patients receive timely, well-coordinated care without unnecessary delays. Our Patient Care Coordinators, or PCCs, serve as a direct link between physicians, patients, and our office in order to simplify communication and provide support.
Physicians can submit a referral using one of two online forms, the Physician’s form or the Self-Referral form. Once processed, the PCCs will work to obtain all necessary records, verify insurance, and secure authorization.
Medical records will then be reviewed in order to ensure patients are promptly scheduled. Once the appointment is set, details will be reviewed and confirmed with the patient, followed by a reminder call the day before their visit.
